What is
Structured Family Care?
Structured Family Caregiving is a Medicaid-funded program in Indiana that allows a family caregiver and the person they care for to live in the same home. As the caregiver, you provide daily support for Activities of Daily Living (ADLs).
This service is designed for individuals who qualify for Indiana’s Health & Wellness, PathWays, or Traumatic Brain Injury (TBI) waivers, and need consistent support at home.
Activities of Daily Living (ADLs) can include:
- Bathing, grooming, dressing, toileting, and mobility
- Meal preparation, housekeeping, and transportation
- Accompanying the recipient to medical appointments and community outings
Structured Family Caregiving with Guardian Care offers more than just pay: it provides stability, resources, and a built-in support system for caregivers.
Eligibility
You may qualify for attendant care or personal care services through Indiana’s H&W/PathWays Medicaid waiver if:
✅ You are an Indiana resident and equire assistance with Activities of Daily Living (ADLs)
✅ You live with your caregiver
